When the Surveyor Asks for the Calibration Log
The call came three weeks out: an announced Joint Commission survey, first one in two years for this client hospital. The biomed manager pulled the binder and found half the infusion pump entries missing a measured value — just a checkmark under "pass." No reference standard listed. No instrument ID. A surveyor asking "what did you measure, against what, and with what" would get a shrug.
That gap is common. Most shops track that a device got calibrated. Far fewer consistently capture what was measured, against what standard, with what instrument, and by whom — the four things that turn a checkmark into evidence.
A medical equipment calibration log template exists to close exactly that gap. Not as a formality, but as the record a technician, a shop owner, or a surveyor can actually use to answer the obvious follow-up question.

What a Calibration Log Has to Prove
A calibration event is a comparison: a measured value from the device under test, checked against a reference standard, evaluated against a manufacturer-specified tolerance. The record has to preserve all three parts of that comparison, not just the outcome.
A log that only records "calibrated — pass" throws away the comparison and keeps only the conclusion. If a surveyor, a client, or a future technician wants to know how close the device was running to its limit, or what instrument produced the reading, a pass/fail flag alone can't answer that. The measured value and the limit have to sit side by side in the same row.
ANSI/AAMI EQ56, the recommended practice for a medical equipment management program, applies to any organization managing equipment used in routine patient care — explicitly including independent service organizations, not only in-house hospital departments. A calibration log is one artifact inside that broader management program, not a substitute for it.
The Fields a Medical Equipment Calibration Log Template Needs
At minimum, a usable calibration log template captures these fields for every row:
Asset identification. Equipment ID or asset tag, make, model, and serial number. Without a stable asset ID, a device's calibration history scatters across multiple partial records instead of accumulating in one place.
Client or department. For an independent service organization working across multiple hospital clients, this field is what makes a single roster of technicians produce a clean, per-client export later. Without it, one shop's combined log can't be split apart at audit time without manual sorting.
Date of calibration and next-due date. The interval between the two should trace back to the manufacturer's specified schedule, or to a documented alternate equipment maintenance rationale where one applies.
Reference standard used. The specific standard or calibrated reference device the technician compared the equipment against. This is the field most logs skip, and the one a surveyor is most likely to ask about directly.
Instrument used, with its own calibration status. The multimeter, analyzer, or test set that produced the measurement is only as trustworthy as its own current calibration. The log should record which instrument was used and confirm that instrument's calibration was current on the date of use.
Measured value. The actual reading — not just "within spec," the number itself.
Acceptable limit or tolerance. The manufacturer-specified range the measured value was checked against. Recording the limit next to the measured value is what lets anyone, later, independently verify the pass/fail call.
Pass/fail result. The outcome, following directly from the measured value and the limit already on the row — never standing alone.
Corrective action, if failed. What was adjusted, replaced, or escalated, and the date the device was returned to service.
Technician identification and sign-off. Name or ID, plus a signature or initials confirming the technician personally performed and stands behind the entry.

Reference Standards and Instrument Traceability
The reference-standard field is where most homegrown logs fall short, because it requires a second layer of recordkeeping: the calibration status of the test instrument itself.
An instrument that is out of its own calibration window produces measurements that cannot be trusted, regardless of how carefully the technician recorded them. A rigorous calibration log template treats the test instrument as a piece of equipment with its own due date, not just a tool sitting in a technician's bag.
In practice this means the log should let a reviewer trace a chain: measured value → instrument used → that instrument's own calibration date. If any link in that chain is missing, the entry doesn't prove what it claims to prove.
This traceability is also what distinguishes a calibration log from a general service history log for medical equipment. A service history log tracks everything that happened to a device over its life — repairs, parts replaced, PM visits. A calibration log is narrower and more exacting: it exists specifically to document a measurement against a standard.
Where This Log Ends and the Electrical Safety Log Begins
Calibration and electrical safety testing get conflated often, but they check different things. Calibration confirms a device measures or delivers what it claims to. Electrical safety testing confirms the device won't create a shock or leakage hazard for a patient or operator.
Both produce similar-looking rows — a measured value, a limit, a pass/fail, a technician sign-off — which is why it's tempting to merge them into one log. In practice, keeping them as separate logs (even if stored in the same binder or platform) makes each one easier to audit on its own terms, and avoids a surveyor having to sort calibration entries out from leakage-current entries mixed into the same table.
