The Night Before the Survey: Loose Readings, No Story
A technician calibrated an infusion pump six months ago. The reading is written on a service tag, taped inside the pump's battery door. Another reading from the same shift lives in a notebook back at the shop. A third sits in a spreadsheet a former employee built and nobody has touched since.
When a surveyor asks to see the calibration history on that pump, none of those three sources tell the whole story by itself. The tag has a number but no reference standard. The notebook has a date but no technician initials. The spreadsheet has a device ID but the reading was never transcribed into it.
This is not a rare failure. It is the default state for any shop that treats calibration as an event instead of a record. The reading gets taken correctly. It just never gets captured in a form that answers the next question a surveyor asks: what did you measure it against, what number did you get, and who is accountable for that number.
An equipment calibration log template fixes the capture problem, not the calibration problem. This article walks through what fields belong in that template, how to fill one out correctly, and how a single filled row becomes part of a defensible, ongoing record rather than a one-off note.
What a Calibration Log Actually Needs to Capture
A calibration reading by itself is just a number. What makes it useful later is the context around it. A usable equipment calibration log template needs to answer six questions for every entry, without requiring the reader to go find a second document:
- Which device, by asset ID and serial number, not just a description.
- What interval this calibration falls on — the schedule the device is supposed to follow.
- What reference standard was used to take the measurement.
- What value was measured, in the actual unit of the test.
- What limit or tolerance that value was checked against, and who set it.
- Who performed the check, on what date, and whether the result was pass or fail.
Miss any one of these and the entry stops being self-contained. A measured value with no stated tolerance tells a reviewer nothing about whether the device passed. A pass/fail mark with no measured value behind it is an assertion, not a record.

Building the Equipment Calibration Log Template Field by Field
Start with columns, not a form. A form is easy to fill out once and hard to search across a hundred devices. A row-based log — one row per calibration event — is what turns into a report a surveyor can actually read.
Asset identifier. Use the same ID your work orders use. If your shop tags equipment with a barcode or asset number, that number goes here, not the device model name alone. Model names repeat across a fleet; asset numbers should not.
Interval. Record the assigned calibration interval for that device — monthly, quarterly, annual, or whatever your program specifies for that equipment class. This is what lets you compute a next-due date, and it is the field most homegrown spreadsheets skip.
Reference standard. Name the calibrated instrument or reference source used to take the measurement, including its own ID and, if relevant, its own current calibration date. A measurement is only as good as the standard it was checked against.
Measured value. The actual number your technician read off the device, in the unit specified by the test procedure. Do not round this to "pass" before it hits the log — the raw value is the record; the pass/fail judgment is a separate field.
Limit or tolerance. The acceptable range or threshold for that device, as defined by your program or the manufacturer's service documentation. This is where your shop states, in writing, what "in tolerance" means for this specific piece of equipment — and where a surveyor will look to see whether your pass/fail calls line up with a documented standard, not a technician's memory.
Technician and date. Full name or initials tied to a roster, plus the date the reading was taken. If your shop serves several client sites, this field should also carry which client the equipment belongs to, so the same technician roster produces a clean, separable record per site.
Result. Pass, fail, or adjusted-and-retested, with a short note if the device required correction before it passed.
A Worked Example: Reading Against Your Own Limit
Here is how those fields work together on a single entry, using a generic example. Say a shop calibrates a patient monitor's non-invasive blood pressure module against a reference pressure source.
The reference standard reads 150 mmHg. The device under test displays 147 mmHg. The shop's documented tolerance for that device class, drawn from the manufacturer's service manual, is ±5 mmHg. The measured deviation is 3 mmHg, which falls inside the ±5 mmHg band, so the entry is logged as a pass.
Notice what makes this defensible: the tolerance did not come from the technician's judgment at the moment of the test. It was documented ahead of time, tied to that device class, and the measured value is checked against it in the log itself. Change the tolerance to ±2 mmHg for a different device class and the same 3 mmHg deviation becomes a fail — the arithmetic is simple, but it only works if the limit is written down before the reading is taken, not decided after.
This is the pattern to repeat for any parameter your shop calibrates: capture the reference reading, the device reading, the documented limit, and let the comparison — not a technician's recollection — determine the result. The specific tolerance for any given device or test always belongs to the manufacturer's documentation or your shop's own written program; confirm it there rather than assuming a single number applies across equipment types.

A single filled-in log row is useful for one device. A shop's real problem is usually hundreds of devices across several client sites, each on its own interval, each needing its next due date tracked without a technician manually paging through a binder.

Calibration and PM schedules are related but not identical — a device can be within its PM interval and still be due for calibration, or the reverse.
