When the Binder Comes Up Short
A hospital client calls two days before an announced survey. Their internal audit found electrical safety records for eleven infusion pumps with no leakage current values entered — just a checkmark under "pass." The tech who ran those tests six months ago is on vacation. Nobody can say which analyzer was used, what limit it was tested against, or whether the reading was 40 microamps or 340.
This is the failure mode an electrical safety test log template exists to prevent. Not because a checkmark is dishonest, but because a checkmark carries no information a surveyor, a client, or a future technician can act on. The value of the record is in the numbers, the date, the equipment identifier, and the limit those numbers were measured against — not the pass/fail label alone.

What an Electrical Safety Test Log Template Actually Records
An electrical safety test log template is not a checklist of yes/no items. It is a structured record of measured values, tied to a specific device, a specific analyzer, a specific technician, and a specific date, with enough context that a second reader can reconstruct what happened without asking anyone.
At minimum, a usable log includes:
- Facility and department, plus the client name if the equipment belongs to a hospital or clinic served under contract
- Device identification: manufacturer, model, serial number, and the internal asset tag or barcode used to track the unit
- Test date and the technician's name or initials
- The analyzer used, including its own calibration due date
- Ground resistance reading
- Chassis leakage current reading
- Lead-to-ground (and, where applicable, lead-to-lead) leakage current reading
- Insulation resistance or polarity check, where the device or standard calls for it
- The limit each reading was tested against, and the pass/fail determination
- Notes on any deviation, repeat test, or corrective action taken
Leave out the identifying fields and the numbers become orphaned data — accurate, maybe, but useless to anyone reconstructing the record later.
Ground Resistance: The First Line Item
Ground resistance testing confirms that a device's chassis is properly bonded to its power cord's ground conductor. A log entry needs the measured resistance value, the analyzer's applied test current if the analyzer reports it, and the limit that value was checked against.
Skip the raw number and record only "pass," and a surveyor — or your own quality team six months later — has no way to tell whether the reading was comfortably under the limit or a hair's width from failing. Both circumstances currently show the same word on the page.
Leakage Current: Chassis and Lead Fields
Chassis leakage and lead leakage are usually the two measurements a surveyor asks about first, because they speak directly to patient and operator safety. A complete electrical safety test log template separates these into distinct fields rather than folding them into one generic "leakage" line, since a device can pass one and fail the other.
Record the actual microamp reading for each, along with which condition it was measured under — normal condition versus a single-fault condition, if your analyzer and procedure test both. The specific microamp limit that applies to a given device depends on its patient-contact classification and the standard your facility has adopted. This article intentionally does not assert a single number as universal — confirm the applicable limit against NFPA 99, the device manufacturer's documentation, and any state-specific requirement before treating it as a pass/fail threshold. Your log template should have a field for that limit so it travels with the record, not just in a procedure binder somewhere else.
Insulation Resistance, Polarity, and the Fields Techs Skip
Ground resistance and leakage current get the attention, but a thorough electrical safety test log template also has room for polarity checks, receptacle testing where applicable, and insulation resistance on devices where that measurement is part of the manufacturer's specified procedure.
These fields are the ones most often left blank under time pressure. A log template that includes them as required entries, rather than optional notes, is more likely to get them filled in consistently across a shop with multiple technicians and varying experience levels.
Pass/Fail Logic: Recording Against a Limit, Not a Number
The most common weakness in a homemade log is recording a pass/fail column without the limit the reading was measured against. A well-built electrical safety test log template treats the limit as a field of its own — sourced from the applicable standard, the device manufacturer, or your facility's policy — rather than a number baked into the form and left unchanged as devices, standards, or facility policy shift.
This is worth building as a habit even before a template forces the issue. When you enter both the reading and the limit side by side, the pass/fail determination becomes something anyone reviewing the record later can independently verify, instead of something they have to take on faith.
Documentation Aid, Not Compliance Advice
An electrical safety test log template is a documentation aid. It is not legal, regulatory, or accreditation advice, and using one does not transfer your compliance responsibility to the template or to the software that stores it. You remain responsible for confirming the limits, frequencies, and procedures that apply to your equipment and your accrediting body.
