A surveyor's visit is announced for Thursday. Wednesday night, the biomed lead is pulling files for every ventilator on three nursing units. Two PM stickers are current. One is six weeks overdue. A fourth unit isn't in the spreadsheet at all — it came in as a loaner and nobody logged it.
This is the moment a ventilator preventive maintenance checklist earns its place in a service record, not a drawer. Ventilators are life-support equipment. A missing or incomplete PM record on one unit can turn a routine equipment-management review into a pointed conversation about the whole program.
Most shops already know the individual steps: check the flow sensor, test the alarms, verify battery runtime. The harder part is having those steps written down in a consistent, repeatable format, tied to a specific asset, with a signature and a date, so the record itself does the explaining when someone asks for it.

What a Ventilator Preventive Maintenance Checklist Should Cover
A ventilator preventive maintenance checklist is only useful if it maps to what actually fails on these devices in the field. At minimum, a defensible checklist covers six categories.
Visual and mechanical inspection. Housing integrity, tubing and connector wear, casters and mounting hardware, power cord condition, and cleanliness of intake filters. Document any physical damage before it becomes a functional problem.
Flow and pressure verification. Using a calibrated test lung or flow analyzer, confirm delivered tidal volume, peak inspiratory pressure, and PEEP against the ventilator's own displayed values, within the tolerance specified in the manufacturer's service manual. This is the core functional check on a ventilator preventive maintenance checklist, and it should be logged as a measured value, not a pass/fail checkbox alone.
Alarm verification. Trigger and confirm every safety alarm the unit supports — low pressure, high pressure, apnea, low battery, disconnect — and record that each alarm both sounded and displayed correctly.
Electrical safety testing. Chassis and enclosure leakage current is measured against the same general thresholds used across biomedical equipment: 300 µA for general patient-care areas and 100 µA for critical-care areas, under normal conditions. As a worked example: if a bench meter reads 65 µA on a ventilator destined for a general medical floor, that unit passes against the 300 µA threshold with margin; the same reading on a unit going to an ICU bed is closer to — but still under — the tighter 100 µA critical-care limit, and should be flagged for a repeat check at the next interval rather than assumed stable. Battery and backup power. Runtime under load, charge indicator accuracy, and behavior on simulated power loss.
Filters and consumables. Bacterial/viral filter replacement per the manufacturer's replacement interval, and inspection of internal cooling filters where accessible.
Each category should have its own line item on the checklist, with space for a measured value, a pass/fail determination, and the technician's initials — not a single generic "PM complete" checkbox.
Setting the PM Interval — and Where Ventilators Fit Relative to AEM
Most hospitals set ventilator PM intervals directly from the manufacturer's service manual. Under the Medicare Conditions of Participation at 42 CFR 482.41, hospital facilities, supplies, and equipment must be maintained to ensure an acceptable level of safety and quality. CMS guidance at 42 CFR 482.41(c)(2), detailed further in CMS Survey and Certification memo 14-07, allows hospitals to follow either manufacturer-recommended intervals or a documented Alternative Equipment Maintenance (AEM) program, with the safety determination made by qualified personnel.
That AEM flexibility has explicit carve-outs: imaging and radiologic equipment, medical lasers, equipment with a maintenance requirement imposed by federal, state, or local law, and new equipment lacking sufficient maintenance history are excluded from AEM treatment. Ventilators are not named on that exclusion list in CMS guidance, but because they are life-support devices, many programs choose to keep them on manufacturer-recommended intervals rather than extend them under AEM. Whether your program applies AEM to a given ventilator model is a decision for your qualified personnel, made in writing, and it is worth confirming your approach directly with your accrediting organization before you rely on it.
Critical access hospitals work from a parallel framework: 42 CFR 485.623(b)(1), with CMS Survey and Certification memo 14-41, requires essential mechanical, electrical, and patient-care equipment be maintained in safe operating condition, with similar latitude to adjust frequency through a documented AEM approach.
None of this is a substitute for reading your own equipment's service manual. It is the regulatory backdrop that explains why the interval you chose needs to be written down and justified, not just followed.
Documentation That Survives an Audit
A checklist only protects you if the completed record can stand on its own. For each ventilator PM, the record should show:
- Asset ID, serial number, and location at time of service
- Date of service and technician identity
- Each checklist category above, with measured values where applicable (not just checkmarks)
- Pass/fail determination and any corrective action taken
- Next due date, calculated from the interval your program has documented
