A surveyor points at a ventilator on the unit and asks for its maintenance history for the past year. Someone goes to find the binder. The entries are there, mostly — a date, a checkmark, initials that could belong to two different technicians. No reading recorded. No indication of what was actually tested. The surveyor asks a second question, and now it's a guessing game about who serviced this unit and what they found.
This is the moment a maintenance log either holds up or doesn't. A checkmark says a technician was in the room. It doesn't say what they measured, what passed, what failed, or what they did about it. Surveyors and internal auditors are trained to ask the second question.
Most shops don't have a bad process — they have an incomplete record of a good process. The PM happened. The leakage test happened. The calibration was checked against tolerance. None of that made it onto paper in a form that answers a follow-up question six months later.

What a defensible medical equipment maintenance log template actually records
A maintenance log is not a to-do list. It's evidence that a specific task was performed, on a specific device, by a specific person, on a specific date, with a specific result. Strip out any one of those five elements and the record stops answering the question a surveyor or auditor is actually asking: did this happen, and can you prove it happened correctly.
That means a usable medical equipment maintenance log template is built around outcomes, not just tasks. "PM completed" is a task. "Leakage current measured at 42 µA, within limit" is an outcome. The second version is what turns a log into a record instead of a checklist.
This distinction is the difference between a log that survives a records request and one that generates more questions than it answers.
Field-by-field: what belongs in each entry
A minimum defensible entry includes:
Device identification. Manufacturer, model, and a unique asset or serial number — not a department name or a bed number, which change. If the device carries a UDI, capturing it ties the log entry to the FDA's device identity system rather than an internal label that might not match a report six months later.
Task performed. Name the specific procedure — not "PM," but "annual PM per manufacturer schedule" or "post-repair function check." Vague task names are the single most common reason a log entry fails to answer a follow-up question.
Reading or measurement, where applicable. If the task involves a number — a leakage current, a calibration deviation, a battery run-time — record the actual value, not just pass/fail. A worked example: a general-care patient monitor tests at 180 µA against a chassis leakage limit of 300 µA under normal conditions. That's a pass, with margin. A ventilator in a critical-care area testing at 85 µA against the tighter 100 µA critical-care threshold is also a pass, but by a much narrower margin worth flagging for the next interval. The number tells a story a checkmark can't.
Result and disposition. Pass, fail, or fail-with-corrective-action, and what was done about it. A failed test with no documented follow-up is worse than no test at all, because it shows the deficiency was known and left unresolved.
Technician identity and credential. Full name or employee ID, not initials. If your shop tracks CBET certification for technicians — a credential AAMI offers through an accredited pathway — noting it on the record supports a competency question a surveyor may ask separately from the equipment question.
Date performed and next-due date. The completion date alone doesn't establish whether the shop is on schedule. The forward-looking due date is what proves a program, not a one-time event.
Sign-off. A named, dated signature or its digital equivalent. This is the field most often missing on paper logs, and it's the one that turns a set of notes into an accountable record.
Miss any of these and the entry can be technically true and still fail to hold up. "Tech was there" is not the same claim as "task was performed, here's the reading, here's who's accountable for it."
Where paper and spreadsheet logs break down
Paper binders and spreadsheet logs don't fail because the data is wrong. They fail because the format can't enforce completeness. A technician can skip the reading field on paper with nothing stopping them. A spreadsheet column can go blank for months before anyone notices the gap. Neither format flags a missed technician signature until someone is already searching for it under time pressure.
The other common failure is fragmentation across sites. An independent service organization running PM programs at several client hospitals often ends up with a different binder — or a different spreadsheet tab — per site. When a client asks for a single device's full service history, someone has to reconstruct it by hand across files that don't talk to each other.
None of this means the underlying maintenance work was bad. It means the record of the work wasn't structured to survive a request it wasn't designed for.
Building or buying a log template that holds up
A shop can build this structure into a spreadsheet themselves — a column for each field above, a validation rule that won't let a row save without a reading and a sign-off, one tab per device or per client. It works, and plenty of shops run this way successfully.
The tradeoff is time and consistency. Every new technician has to learn the format. Every new client contract means another tab or file to maintain.
